    Gold plated chains/charms/connectors
    Chains/charms/connectors breaking-lost, damaged, or tarnishing.

    Covered:

    Luxe Collection Charms and Connectors:

    Warranty on metal ring tarnishing
    Warranty on material breaking (not due to damage or accidents)
    Replacement provided for covered issues
    Up to three free replacements; thereafter, the replacement will cost as much as it’s equivalent at that time.

    GemYou Chains:

    Tarnishing within 24-48 hours of wear (one-time replacement with a chain of the same price)
    Breaking of the chain (loss is not covered)
    Breaking at the welding point
    Up to three free repairs; thereafter, repairs cost $15
    One-time chain replacement available

    Limitations:

    The warranty does not cover wear and tear or damage resulting from misuse, accidents, or neglect.

    To initiate a warranty claim, customers should provide proof of purchase, along with photographs of the damaged item. This helps in assessing the warranty coverage quickly.

    1. Final Sale Policy:
      • I acknowledge that all sales are considered final sale.

      • I understand that there are no refunds for broken jewelry; however, I will have the option for repairs on broken pieces.

      • I note that all sales are final, as indicated on the price list.

    2. Understanding of Permanent Jewelry:
      • I confirm that all my questions regarding permanent jewelry have been answered by GemYourself! LLC to my satisfaction.

      • I understand that the jewelry can break and/or stretch if pulled or caught.

      • I understand that this permanent jewelry can easily be removed.

      • I acknowledge that I have been informed about the proper care required for my permanent jewelry.

    3. Guarantee Period:
      • I understand that if anything happens to my jewelry as described under warranty, GemYourself! LLC will fix or replace it at their next pop-up.

      • I acknowledge that while the weld is guaranteed, the chain or jewelry itself is not.

    4. Health and Safety Acknowledgment:
      • I confirm that I do not have a pacemaker, any sensitivity or allergy to metals, or any other medical condition that could make this permanent jewelry experience unsafe.

      • I understand the importance of this information for my safety and the safety of the technicians.

    5. Liability Release:
      • By signing this form, I acknowledge and accept that GemYourself! LLC is not liable for any medical condition or injury caused by this experience or jewelry.

      • I release GemYourself! LLC from any liability for complications, injuries, or damages resulting from the procedure.

    6. Photography and Media Consent:
      • I grant GemYourself! LLC permission to capture images and likeness in photographs, videotapes, and/or digital media during the application process for promotional and marketing purposes.

    7. Understanding of Risks and Procedure:
      • I acknowledge that I have discussed the procedure and its potential risks with the practitioner.

      • I understand that the procedure may involve pain, swelling, discomfort, infection, scarring, allergic reactions, migration, or rejection of the jewelry.

      • I have been provided with aftercare instructions and understand their importance in minimizing complications.

    8. Age Verification:
      • I confirm that I am at least 18 years of age, or if I am a minor, I have obtained the consent of my parent or legal guardian for this procedure.
